  • You may be eligible for deductible assistance up to $200 per treatment if applicable
If eligible, you may enroll in the Heron Connect® Patient Assistance Program

  • SUSTOL can be provided at no cost if you’re dealing with financial hardships, and if you meet the program eligibility criteria

  • Limitations apply. Offer not valid as follows: (a) patients covered under Medicare, Medicaid, or any federal or state program; (b) where plan covers treatment for the patient for the entire cost of the prescription drug. Patients pay $0 per copay per dose per 12-month calendar period. During the eligibility period, the out-of-pocket drug expenses for CINVANTI® (aprepitant) injectable emulsion and/or SUSTOL will be covered up to $80 per CINVANTI (aprepitant) treatment or $160 per SUSTOL (granisetron) treatment. When applicable, deductible assistance up to $200 per treatment will be covered. For cash paying patients, the program will cover $150 per prescription up to $1,800 per benefit cap per calendar year. Eligibility for the copay assistance is for 12 months, after which the patient will need to reapply for continued assistance.
